The REGISTRATION FORM will register your students with the School of Art for the studio sessions. That address is:

The FEE PAYMENT FORM must be sent to the Business Office along with the fifteen dollars ($15) per student fee. That address is:

The Business Office will send a receipt to the School of Art as proof of your payment. Please be sure to include your FAX number for confirmation purposes.

Make copies of each form, as needed. Forms are due by Friday, November 7, 2008.

Cancellation: If you must, please cancel by Friday, November 7, 2008.
